11a. 一般環(huán)境健康安全（General Environmental Health and Safety (EHS) ）
11.1 受核工廠應(yīng)有一套內(nèi)容包括環(huán)境衛(wèi)生安全和工作條件政策和程序的環(huán)境衛(wèi)生與安全計劃書：The facility should have a written EHS program that includes policies and procedures for environmental, health, safety (EHS) and working conditions.
11.2 受核工廠用當?shù)卣Z文制訂的環(huán)境衛(wèi)生安全政策應(yīng)可供全體雇員索閱：Facility’s written EHS policies should be available to all employees in the local language.
11.3 工廠須指派一名環(huán)境衛(wèi)生與安全事務(wù)統(tǒng)籌員：An EHS coordinator should has been designated.
11.4 受核工廠須有一個負責環(huán)境衛(wèi)生與安全事務(wù)的委員會/團隊：The facility should have an EHS committee/team.
11.5 受核工廠須有關(guān)于工作場所環(huán)境衛(wèi)生安全意識的推廣活動：The facility should promote workplace EHS awareness.
11.6 所有雇員在開始執(zhí)行新職務(wù)之前，都應(yīng)接受與新工作有關(guān)的環(huán)境衛(wèi)生安全培訓/教育：Job-specific EHS training/education should be provided to all employees prior to starting a new job.
11.7 所有雇員都應(yīng)有機會就環(huán)境衛(wèi)生安全問題提出意見/表達關(guān)注：All employees should be given the opportunity to voice EHS opinions/concerns.
11.8 應(yīng)對擬送進受核工廠的新組件、新物料、新化學品、新機器、新產(chǎn)品實施管制，以防對環(huán)境衛(wèi)生安全有潛在損害的這些東西未經(jīng)檢驗進入：The controls should be in place to prevent new components, materials, chemicals, machinery, and products from entering the facility, which have not been reviewed for potential EHS hazards.
11.9 送入受核工廠的容器均應(yīng)貼附了圖文清晰的標貼：The containers which coming into the facility should have been legibly affixed labels.
11.10 應(yīng)向承包商的人員講解作業(yè)場地環(huán)境衛(wèi)生安全規(guī)則，并安排他們認識廠內(nèi)的環(huán)境：Contractor personnel should be given site EHS rules and orientation.
11.11 全體雇員均應(yīng)接受過化學品安全培訓：The employees should have been trained on the subject of chemical safety.
11.12 所生產(chǎn)的、進口的或使用的化學品都應(yīng)有最新的物料安全資料表（MSDS），而且都是用當?shù)卣Z文編寫的，可供全體雇員索閱：Up-to-date MSDS’s should be present for all chemical produced, imported, or used, and they should be available to all employees in the local language.
11.13 所生產(chǎn)的、進口的和/或在作業(yè)場地上使用的危險化學品，應(yīng)全部列入當進的庫存化學品目錄中：A current chemical inventory should be available for all hazardous chemicals produced, imported, and/or used at the site.
11.14 在處理和使用腐蝕性化學品或大量溶劑的地方須設(shè)置功能正常的緊急洗眼站和淋浴設(shè)備：Functional emergency eyewash station and showers should be provided where corrosive chemicals or high volumes of solvents are handled and used.
11.15 盛載著危險化學品的儲存箱和加工箱、喉管、閥等均須附加標簽和/或色標：Storage and process tanks, piping, and valves containing hazardous chemicals should be labeled and/or color-coded.
11b. 健康與安全（Health and Safety）
11.16 所有已經(jīng)上報的意外事故和幾乎釀成意外事故的情況都要調(diào)查：All reported accidents and near misses should be investigated.
a) 使用調(diào)查表格調(diào)查意外事故/幾乎釀成意外事故，并完整地填寫表格：Accident/near miss investigation form used and filled out completely.
b) 指令并鼓勵雇員報告意外事故和幾乎釀成意外事故：Employees instructed and encouraged to report accidents and near misses.
c) 跟進糾正/預防行動，直至這些行動完成：Corrective/preventive actions tracked to completion.
11.17 保證上報意外事故/幾乎釀成意外事故的雇員不受到歧視：The factory should ensure employees that reporting accidents against discrimination.
11.18 穿著松身衣物、身上吊掛飾物外露和頭披長發(fā)的人士一概不得接近開動的機器：Loose clothing, jewelry, and long hair should be prohibited near moving machinery.
11.19 全體雇員都須受過關(guān)于機器安全防護的培訓：The employees should have been trained on the subject of machine safeguarding.
11.20 受核工廠須提供下列受用的機器安全防護裝置：操作點安全防護裝置、電力傳輸安全防護裝置、其它危險動件安全防護裝置、滑輪和傳動帶安全防護裝置、風扇葉安全防護網(wǎng)罩（網(wǎng)隙大小有一定限制）：Machine safeguards should be provided and used: Points of operation, Power transmission devices, other hazardous moving parts, Pulleys and belts, Fan blades protected with a guard and have limited openings.
11.21 應(yīng)有足夠合適的開機/停機控制器：提供給每一名操作員、可防意外起動、控制器附有標貼和/或有適當?shù)纳珮恕⒉捎眉t色掌壓式/菇狀緊急停機控制器：Starting/stopping controls should be adequate: Provided for each operator, Protected from accidental activation, Labeled and/or properly color-coded, Emergency stops of the palm/mushroom type and colored red.
11.22 凡上文沒有提及的其它機器、設(shè)備和各種固定裝置，操作狀態(tài)均須安全：Any machines, equipment, and fixtures not covered above should be in safe operating condition.
11.23 在使用涂料和化學品（例如溶劑、焊藥、粉塵）的地方須設(shè)置足夠合適的局部廢氣抽排裝置：Adequate local exhaust ventilation present where paints and chemicals (e.g., solvents, solder, dust) should be used.
11.24 任何人員暴露于危害物質(zhì)，暴露劑量均維持不超過最低安全標準：All personnel exposures should be maintained at or below safe exposure levels.
11.25 受核工廠應(yīng)有一套關(guān)于危害能源管制的程序文件：The facility should have a written procedure for the control of hazardous energy.
11.26 須為所有經(jīng)授權(quán)使用涂料/化學品的人員、受影響雇員或其它受危害能源管制程序影響的雇員提供控制程序培訓：Training should be provided to all authorized users, affected employees, or other employees who are impacted by the hazardous energy control procedure.
11.27 須有足夠的防擾鎖具和防干擾掛簽等隔離設(shè)備可供使用：鎖具、掛簽、專組鎖箱、電箱斷路器鎖定裝置、閥門鎖定裝置、搭扣：Sufficient lockout and tagout isolation equipment should be available: Locks, Tags, Group lock box, Circuit Breaker lockouts, Valve lockouts, Hasps.
11.28 在對設(shè)備和機器進行維修保養(yǎng)之前，首先應(yīng)把輸往這些設(shè)備/機器的能源鎖斷：The energy to equipment and machinery should be locked out prior to service or maintenance.
11.29 須完成一項危害因素評估工作，并保存了有關(guān)的記錄文件，確定每一項作業(yè)場需要的個人防護設(shè)備：A documented hazard assessment should has been completed which identifies the personal protective equipment (PPE) required for each operation.
11.30 雇員/訪客須獲得提供并依規(guī)定使用下列器具：眼部/面部防護器具、腳部防護器具、聽力保護器具、手部防護器具、頭部防護器具、防護衣物、呼吸系統(tǒng)保護器具、供訪客用的個人防護器具：The following should be provided and used: Eye/face protection, Foot Protection, Hearing Protection, Hand Protection, Head Protection, Protective Clothing, Respiratory Protection, PPE Provided To Visitors.
11.31 在使用個人防護設(shè)備方面，雇員都須經(jīng)過培訓：The employees should have been trained on the subject of personal protective equipment.
11.32 凡須穿著或佩帶個人防護設(shè)備的地方都須張貼有關(guān)的標志：Signs should be posted in all areas where personal protective equipment must be worn.
11.33 經(jīng)授權(quán)操作起重機/吊機的人員都須經(jīng)過有關(guān)工作的安全培訓：The authorized crane/hoist operators should be received job-specific safety training.
11.34 規(guī)定要對起重機、吊機、吊索進行安全檢查：Inspections should be conducted on cranes, hoists, and slings.
11.35 起重機、吊索和吊機上應(yīng)清楚標示載重容限：Capacity limits should be clearly marked on cranes, slings, and hoists.
11.36 樓梯都應(yīng)設(shè)有扶手和中高欄桿：Stairs should be provided with standard handrails and midrails.
11.37 凡開放式樓梯、平臺和升起的地面，所有開放邊都應(yīng)設(shè)有護欄：Open stairs, platforms, and elevated floors should be guarded by railings on open sides.
11.38 需要工人在升起的工作平臺上執(zhí)行任務(wù)時，應(yīng)只使用經(jīng)認可的工作平臺：Only approved working platforms are used when employees are elevated to perform work.
11.39 升起的工作平臺（即吊籠）應(yīng)穩(wěn)妥地同升提設(shè)備（即鏟車、高升程裝置等等）在一起：Elevated work platforms (i.e., baskets), should be secured to the lifting device (i.e., forklift, high lift, etc).
11.40 所有梯子均須在良好的使用狀態(tài)：All ladders should be in good condition for use.
11.41 地面上和屋頂上的開口均應(yīng)予以覆蓋或防護，以免發(fā)生人員或?qū)ο笙聣嬍录篎loor and roof openings should be covered or protected to prevent falls.
11.42 受核工廠應(yīng)有一套關(guān)于進入密閉空間地方的程序文件：The facility should have a written procedure for entry into confined space areas.
11.43 進入密閉空間的雇員須接受有關(guān)進入密閉空間地方的培訓：The employees who enter confined spaces should have been trained on the subject of entry into confined space areas.
11.44 密閉空間應(yīng)有標識和保安措施：儲存缸、處理容器、設(shè)有防堤的儲存缸場、鍋爐、焗爐、地滘、壕溝、輸送管、其它密閉空間：Confined spaces must be identified and secured: Storage tanks, Process vessels, Diked tank farms, Boilers, Ovens, Silos, Underground trenches, Enclosed conveyors, Other.
11.45 經(jīng)授權(quán)操作叉車的人員須接受過有關(guān)工作的安全培訓：The authorized forklift operator should have received forklift safety training.
11.46 貫徹對叉車的日常檢驗和維修，以維持其良好、安全和有效狀態(tài)：Forklifts should be inspected and maintained in good, safe, working condition.
11.47 在有人流的地方都須設(shè)置“停車”標志和鏡子，并展示了限速行車標志：There are “Stop” signs, mirrors, and posted speed limits in areas where pedestrian traffic may be present.
11.48 工人進入貨柜裝卸貨物之前，應(yīng)首先把掛車/貨柜穩(wěn)穩(wěn)固定，以防移動：Trailers/containers should be secured from movement prior to entry for loading and unloading.
11c. 環(huán)境（Environmental）
11.49 有害廢棄物應(yīng)管理得當：Hazardous waste should be managed properly.
11.50 所有有害廢棄物須在經(jīng)政府批準的場地處置：All hazardous waste should be disposed of at a government approved site.
11.51 受核工廠須領(lǐng)有加工廠廢水排放許可證：The facility should have a process waste water discharge permit.
11.52 受核工廠須為所有氣體排放點在當?shù)丨h(huán)保當局辦妥必要的登記手續(xù)：The facility should have obtained the necessary registration from the local environmental agency for all air discharge points.
11.53 為處理生活污水，受核工廠須設(shè)有生活污水處理系統(tǒng)或領(lǐng)有排污許可證：The facility should have a septic system or discharge permit to dispose of sewage.
