SOCIAL COMPLIANCE AUDIT REPORT RELEASE AUTHORIZATION

This REPORT RELEASE AUTHORIZATION (this “Authorization”) is entered into by the
undersigned vendor (“Vendor”) of Office Depot, Inc. (“Office Depot”) as of the date set forth
below (the “Effective Date”). By signing below, Vendor hereby authorizes Office Depot’s
approved Social Compliance Audit Provider set forth below (“Audit Provider”) to disclose
Inspection Data (as defined below) to Office Depot, to other vendors/suppliers sourcing to one
or multiple Office Depot regions, and to other retailers. Furthermore, Vendor hereby releases
Audit Provider from any liability and damages which may result from any such disclosure. As
used herein, “Inspection Data” shall mean any and all information and data compiled by Audit
Provider in connection with an audit of Vendor’s production facilities and/or suppliers, including,
without limitation, any and all information supplied to and generated by Audit Provider during
the course of any such audit for the purpose of confirming compliance with Office Depot’s
requirements as well as any information relating to Vendor’s factories, including without
limitation those set forth below.
This Authorization shall remain in effect from the Effective Date until Vendor provides written
confirmation of its termination to Audit Provider, with a copy to Office Depot at the following
address: Office Depot, Inc., 6600 North Military Trail, Boca Raton, FL 33496, Attn: SA-
COMPLIANCE (Mail Code C416P) or email to [email protected].
IN WITNESS WHEREOF, Vendor has caused its duly authorized representative to execute this
Authorization as of the Effective Date.
